Extract of data from the population register

The population register is one of the four basic registers in Czechia, and is used to share data across public administrations. It contains data on citizens and on foreign nationals with a long-term or permanent residence permit in Czechia.

The population register holds the following information:

  • surname;
  • surname at birth;
  • first name(s);
  • sex;
  • marital status or registered partnership;
  • address of residence;
  • correspondence address;
  • date of birth;
  • place and district/country of birth;
  • limitation of legal capacity;
  • date of death;
  • place and district/country of death;
  • date on which the court decision on the declaration of death became final;
  • citizenship;
  • the numbers and types of identification documents and their expiry date;
  • data box type and identifier;
  • telephone number;
  • e-mail address;
  • qualified certificate for electronic signature (serial number, issuer and validity).

An excerpt of data from the population register means the right of a natural person to request provision of the data kept about them in the population register.

You are a Czech citizen or a foreign national with a long-term or permanent residence permit in Czechia, you are aged 15 years or older, and you wish to obtain an excerpt of the data that is kept about you in the population register.

The following are also entitled to request an excerpt of data from the population register:

  • an authorised representative of a natural person over the age of 15 who is acting on the basis of a special power of attorney with the officially verified signature of the principal,
  • a legal representative acting for a person under the age of 15,
  • a member of a household acting on the basis of a right of representation that has been approved by a court,
  • the guardian of a natural person whose legal capacity has been restricted.

When making a submission in person, you must prove your identity by means of an identification document.

An application sent by post must bear your officially authenticated signature. An officially certified signature is not required if the application is signed before an official of the competent authority.

In your application, please state:

  • your given name and surname;
  • date and place of birth;
  • identification document type and number;
  • address of the place of residence;
  • the scope of the data required and the form in which you would like it to be transmitted.

In person, you can submit an application to the Czech POINT contact point for public administration or at any municipal office of a municipality with extended jurisdiction, regional authority or the Digital and Information Agency. An application submitted in person at the Czech POINT contact point for public administration is dealt with immediately (while you wait), subject to the availability of the population register services.

You can also send an application by post to any municipal authority of a municipality with extended jurisdiction, regional authority or the Digital and Information Agency. This application will be dealt with within 30 days.

Kolik budete platit

The provision of an extract from the population register at the Czech POINT contact point for public administration is subject to the following fee: the first page of the extract CZK 100 and each additional page or part thereof CZK 50. If a multilingual standard form is attached to the extract of data from the population register, the fee of CZK 100 is charged.

The fee for an extract from the population register delivered by a municipal authority with extended jurisdiction, regional authority or the Digital and Information Agency is as follows: for each page or part thereof CZK 50.

Jaký má služba benefit

You will receive an extract of the data kept about you in the population register which will give you an overview of the data held on you in the population register.

In addition, an extract of data from the population register can be used as a document confirming the address of a place of residence. Where a multilingual standard form is attached to this extract, no official translation is required in a Member State of the European Union.
