SERVICE DESCRIPTION

This is an obligation that you have, if there has been a change relating to the information in your certificate of residence permit. 

THIS APPLIES TO YOU, IF there has been a change relating to the information in your residence permit card. Under these circumstances, you are required to report this change to the relevant office of DAMP MOI CR.

SERVICE OUTPUT

The legal obligation is fulfilled by reporting other changes.

WHEN TO USE THE SERVICE

As an EU citizen or as a family member of an EU citizen, you are required to report changes relating to the information in your certificate of residence permit within 15 working days of the day, on which the change occurred.

WHAT YOU NEED IF YOU USE THE SERVICE ELECTRONICALLY

You must report other changes on a duly filled out official form

In order to report this change, you must submit:

  • a document on the change;
  • if there has been a change in the information on the foil-sealed side of the certificate, you must also submit 2 photographs.

WHEN AND HOW TO USE THE SERVICE ELECTRONICALLY

Send your filled out reporting form with the appropriate documents in accordance with the Rules of Administrative Procedure to the official contacts of the MOI by e-mail with a recognised electronic signature, with all attachments being in the format of authorised conversion (via the filing office of the MOI CR), or to the data box (to DB MOI CR).

If a change needs to be made in a residence permit card, you will need to appear in person at the office of the MOI for the purposes of making this change.

HOW MUCH YOU WILL PAY FOR THE ELECTRONIC PROCESSING

Reporting other changes is free of charge provided there is no change to the document.

In other cases, you will pay an administrative fee of CZK 200.

If you report the change electronically, the administrative fee must be paid by bank transfer via the payment details generator on the Information Portal for Foreigners.

WHAT YOU NEED IF YOU USE THE SERVICE IN PERSON OR BY MAIL

You must report other changes on a duly filled out official form.

In order to report this change, you must submit:

  • a document on the change;
  • if there has been a change in the information on the foil-sealed side of the certificate, you must also submit 2 photographs.

WHERE AND HOW OT USE THE SERVICE IN PERSON; WHERE TO SEND THE DOCUMENTS BY MAIL

You must report other changes to the office of the Department for Asylum and Migration Policy of the MOI (DAMP MOI) responsible for the district where your place of residence is situated.

Alternatively, you can send the filled out reporting form, along with the documents, in accordance with the Rules of Administrative Procedure by mail to the official contacts of the MOI or directly to an office of DAMP MOI. It is always necessary to submit the originals or officially authenticated copies of the documents. Do not send the originals of the travel document and the certificate of residence permit by mail.

HOW MUCH YOU WILL PAY FOR USING THE SERVICE IN PERSON OR BY MAIL
