1) Birth of a child who is a citizen of the European Union
If your child is born in the territory of the Czech Republic and it is a citizen of the European Union, there is no need to apply for a residence permit for that child in the Czech Republic. If you want, as a legal guardian you can apply for this child for a:
If the newly-born child stays in the Czech Republic for more than 3 months. In this case, you must pay insurance for the child.
If you are staying in the Czech Republic having a permanent residence permit, you can apply for a permanent residence permit for the child (an EU citizen) on humanitarian grounds. The purpose in this case is family unification with the legal guardian with permanent residence. If you apply within 60 days of the child's birth, the child will be covered by the public health insurance system from the time of its birth to the instant the decision comes into effect. If the application is approved, the child will of course continue to be insured.
If you were unable to apply for reasons outside your control, the 60-day period is understood to be extended until these reasons end.
If you do not manage to submit your application by the deadline, you can still apply for a registration certificate for the newly-born child. You can also apply for a permanent residence permit for a European Union citizen on humanitarian grounds. In this case, the child will only become covered by public health insurance when the permit is issued (when you receive the permit.)
2) Birth of a child who is a family member of an EU citizen, but is not an EU citizen itself
If your child is born in the territory of the Czech Republic, it is entitled to stay temporarily in the territory as long as its legal guardian stays here, but at the longest for the period of 60 days from its birth. If the intention for the child is to stay longer, you have to apply for a:
If the child is staying in the territory together with an EU citizen for a period longer than 60 days, the child's legal guardian must apply for an EU citizen’s family member temporary residence permit for it.

If you file the application by the deadline, the child’s residence is considered to be permanent from the time of its birth until the final decision on the application is taken (when you receive the permit). If you do not wish to apply for a permanent residence for the child, you can, instead, apply for an EU citizen's family member temporary residence permit for the purpose of family unification with its legal guardian who is an EU citizen.

In the case of EU citizens, the deadline is not fixed. For a registration certificate, you can apply (but you do not have to) anytime. For an EU citizen’s permanent residence permit, it is better to apply within 60 days so that the child is covered by the public health insurance system.

By mail, you can only apply for a registration certificate. You can send the filled-in form together with all the requirements by post to the office of the Department for Asylum and Migration Policy of the Ministry of Interior (DAMP MOI) corresponding to your place of residence in the Czech Republic.. A travel document can be sent as a simple copy – you will present its original when you come to pick up the registration certificate.

A foreign national born in the territory of the CZ who is a family member of an EU citizen and is not him/herself an EU citizen is likewise qualified to reside temporarily without a visa/residence permit in the territory of the CZ for the time of his legal guardian’s residence, at most for 60 days from the day of birth.

If a family member of an EU citizen, who is not an EU citizen, wants to reside together with the EU citizen for a period longer than 60 days, the legal guardian (EU citizen) is obliged to apply, on behalf of the newly born foreign national, at the MOI office based on their address for a Temporary residence permit for a family member of an EU citizen.
