The application for a change in entitlement to a disabled person’s pass is used to request the commencement of a procedure to change the entitlement to a disabled person's already granted pass.
The procedure for a change in the entitlement to a disabled person’s pass is initiated by submitting an application on the prescribed form, or also ex officio.
The district social security authority may also initiate the procedure of its own motion if, based on the results of a health assessment, it determines that there are significant facts which are decisive for the entitlement to a disabled person’s pass.

- mobility,
- orientation or
- autism spectrum disorder
who has already been granted a disabled person’s pass.

- you have already been granted entitlement to a disabled person’s pass and
- if there has been change in your disability (mobility, orientation or an autistic spectrum disorder).

What you need if you are using this service
To submit an application for a change in entitlement to a disabled person’s pass in person, you need to fill in the Application for a change in entitlement to a disabled person’s pass form.
For identification purposes, you need to show:
- an identity card,
- a birth certificate for children under 15 and
- a court ruling on the appointment of a legal guardian.

Applications should be submitted at the contact point of the Czech Labour Office according to the place of permanent residence (place of residence) of the individual.

What is the benefit of this service
Persons who hold a:
- TP (severe disability),
- ZTP (extreme disability) or
- ZTP/P (extremely disabled with the need for a guide)
pass have priority in the personal handling of their affairs (with the authorities) and are entitled to public transport benefits (reserved seats, discounts or free transport, a free transport guide/assistant or a guide dog).
Holders of a ZTP or ZTP/P pass and the guides of a holder of a ZTP/P pass may be granted a discount on admission to theatre and film performances, concerts and other cultural and sporting events. There is no automatic entitlement to these benefits.
The procedure to change a disabled person’s pass is initiated on the basis of a written request.
After the application has been submitted, the regional branch of the Czech Labour Office will send a request to the relevant district social security authority for an assessment of the person’s state of health. Based on this assessment, the regional branch of the Czech Labour Office will issue a decision on whether a pass is to be changed or not.
The disabled person’s pass is a public document.
Appeal options
If you are not satisfied with the outcome of a procedure for a change to a disabled person’s pass, you may submit an appeal against the ‘Decision’ within 15 days of receiving that decision.
You can lodge appeals at the regional branch of the Czech Labour Office that decided on the benefit. The Ministry of Labour and Social Affairs will then decide on the appeal.

Sanctions
A natural person commits an offence by intentionally destroying, damaging, altering or misusing a disabled person’s pass.
The holder of a disabled person’s pass commits an offence by:
- failing to fulfil the notification obligation, i.e. not notifying, in writing, the regional branch of the Labour Office that issued the pass, within 8 days of the loss, theft, damage or destruction of the pass, or of any change in the information stated on the pass,
- failing to hand in an existing disabled person’s pass on receiving a new disabled person’s pass; anyone who finds a disabled person’s pass or possesses the pass of a deceased person or a person declared dead must hand it over immediately to any regional branch of the Czech Labour Office. Holders of a disabled person’s pass who have reported the loss of their pass have the same obligation, if they find the card or otherwise recover it. Upon receipt of a new pass, the holder is obliged to hand over the previously found or otherwise recovered pass to the relevant regional branch of the Czech Labour Office. In the event that a new pass is issued in accordance with the law, the holder of a disabled person’s pass must hand over the existing pass to the relevant regional branch of the Czech Labour Office.
A fine of up to CZK 20 000 may be imposed for such offences.
