Submission of data on the self-employed person’s insurance periods and the premiums paid for insurance for each calendar year

A self-employed person may request the district social security administration to send data on the period during which they have been paid sick leave, the period of their participation in pension insurance and the insurance premiums paid for this insurance and the contribution to the state employment policy for each calendar year.

You are self-employed and want to obtain an overview of how long you have been paid sick pay, how long you have been participating in the pension scheme and what contributions you have paid.

You must always request this information in writing from the relevant district social security office, as it keepsrecords for the purposes of pension insurance for self-employed persons who pay pension insurance premiums.

 The competent district social security administration will send you, a self-employed person, the following information on the basis of your request:

  • identification data, i.e. surname, first name, date and place of birth, sex, place of permanent residence, nationality,  
  • personal identification number,
  • the assessment base for determining pension insurance premiums for the relevant reference periods,
  • information on the period for which sick pay was paid,
  • information on the period of participation in the pension scheme,
  • information on pension insurance premiums and state employment policy contributions paid for each calendar year.

You can ask for details of your pension participation and contributions at any time during the year. The District Social Security Administration always provides this information for each calendar year, so the timing of your written request is entirely at your discretion.

To use this service, you must always submit a written request clearly specifying what you are requesting, along with your personal identification number, first name, surname and address or databox ID. The requested data will be sent by post to your address or to your databox.

You can send a request for data on the period of participation of a self-employed person in pension insurance, the period of sickness benefit payments and the insurance premiums paid for each calendar year:

in person to the district social security administration competent according to the place of permanent residence, in Prague to the Prague Social Security Administration and, in Brno, to the Municipal Social Security Administration,

by post directly to the address of the relevant social security administration of your place of residence.
