Preparation of a new personal pension information sheet upon proof of additional periods of insurance or assessment bases - the application is processed by the Czech Social Security Administration

Any person who has been sent an informative personal pension insurance sheet by the Czech Social Security Administration at their request may request a new information sheet if he/she provides proof of additional periods of participation in pension insurance or additional assessment bases or excluded periods in the period after 1985.

You are a person who was sent an informative personal pension insurance sheet by the Czech Social Security Administration upon request, but in which some of your periods of participation in pension insurance or some of your assessment bases or excluded periods in the period after 1985 were not indicated and you have documents proving these missing periods of insurance or missing assessment bases or excluded periods.

You must always ask the Czech Social Security Administration in writing to send you a new pension information sheet. At the same time, you must provide original or certified copies of documents proving missing periods of insurance or missing assessment bases or excluded periods.
 

The Czech Social Security Administration will not issue a new information sheet unless:

• you can only provide additional proof of periods of caring for a child under the age of four, caring for a dependent person or receiving a third-degree invalidity pension (these periods will be decided in the pension award procedure),

• the Czech Social Security Administration doesn’t recognise missing periods of insurance or missing assessment bases or excluded periods as sufficiently proven (these periods or assessment bases will only be decided in the pension award procedure),

• you submit documents for periods falling within the period of the calendar year of the information sheet or the immediately preceding calendar year (these periods will be included in the pension information sheet prepared in the following year(s)).

You can ask for a new personal pension information sheet to be sent to you at any time after you have received the original information sheet, if it does not take into account certain periods of insurance or assessment bases or excluded periods and you have documents to prove them.

The Czech Social Security Administration will send you a new informative personal pension insurance sheet taking into account any additional periods of insurance, assessment bases or excluded periods in the period after 1985 within 90 days from the date of receipt of your request.

To use this service, you must always submit a written request clearly specifying what you are requesting, along with your personal identification number, first name, surname and address or databox ID. You write your application in free form.

This application must be accompanied by original or certified copies of documents proving your additional periods of pension participation or additional assessment bases or excluded periods in the period after 1985.

Step 1

A personal pension information sheet, which does not have a prescribed format, together with original or certified copies of documents can be:

  • sent by post directly to the address of the Czech Social Security Administration.
  • hand it over in person at the locally competent district social security administration, the Prague Social Security Administration or the Municipal Social Security Administration Brno. Here, they will verify the documents proving the unregistered period of pension insurance free of charge and forward them to the Czech Social Security Administration to process your request.

Step 2

The Czech Social Security Administration will check all the details of your application and, in particular, will assess the documents you have submitted to prove additional periods of participation in pension insurance or additional assessment bases or excluded periods.

  • If the new data are sufficiently proven, the Czech Social Security Administration will prepare a new informative personal pension insurance sheet taking into account the newly proven periods of insurance, assessment bases or excluded periods and then send it to the address specified in the application or to your data box within 90 days of the date of receipt of your application.
  • If the Czech Social Security Administration does not consider the new data to be sufficiently proven, it will not issue a new information sheet and will send you a written notificationwithin 90 days, from the date of receipt of your application, stating that the new data cannot be considered sufficiently proven and will therefore be decided upon in the pension award procedure. At the same time, it will return the documents that accompanied your application and that you used to prove the additional periods of insurance, assessment bases or excluded periods.

Even then, the Czech Social Security Administration will not issue a new information sheet:

  • if only periods of caring for a child under four years of age, periods of caring for a person dependent on the care of another person, or periods of receiving a third-degree invalidity pension are additionally proven. You will only receive a written notice from the Czech Social Security Administration that these periods will be decided on in the pension award procedure.
  • if only documents for a period falling within the calendar year of the information sheet or the immediately preceding calendar year are submitted. You will only receive a written communication from the Czech Social Security Administration stating that these periods will be included in the informative personal pension insurance sheet prepared in the following year(s).

Kolik budete platit

A fee is charged for submissions by post according to the valid price list of Czech Post or the postal institution abroad.

Personal submission is free of charge, including verification of documents proving unrecorded periods of insurance.

Jaký má služba benefit

If the missing periods or assessment bases are sufficiently proven, the Czech Social Security Administration is obliged to send you a new information sheet taking into account the additional data.
 

Možnosti odvolání

The Act on the Organisation and Implementation of Social Security, as amended, does not allow for an appeal against an information personal pension sheet. The non-assessment of the period of insurance can only be challenged in the context of the decision on the entitlement to and amount of the pension.
