Reporting the loss, destruction, damage or theft of a residence permit by a third-country national

If you lose, destroy, damage or have your residence document stolen, you must report it to the Ministry of the Interior office.

This applies to you if

You are a third-country national with a long-term visa, long-term or permanent residence and your residence permit is lost, damaged, destroyed or stolen.

Under these conditions, you are obliged to report this event to the relevant Ministry of the Interior office.

When to use this service

You are obliged to report the loss, destruction, damage or theft of a document issued by the Ministry within 3 working days of the date on which the event occurred.

What you need if you are using this service

To apply for a visa duplicate you will need:

• A valid travel document,
• A form for notification of changes,
• In the case of theft, it is recommended to have a confirmation from the police that you have reported the theft of the document.

To apply for a new residence permit card you will need:

• A valid travel document,
• A form for notification of changes,
• The damaged document or the document with non-functional biometric data medium.
• In the case of theft, it is recommended to have a confirmation from the police that you have reported the theft of the document.

Where and how to solve this service

You can apply:

• by Data Box (ID: 6bnaawp), or
• by e-mail with an advanced electronic signature (posta@mv.gov.cz).

All attachments must be in the authorised conversion format (except forms and travel documents).

How much will you pay

• Long-term and permanent residence: 2 500 CZK (1 000 CZK for persons under 15 years of age)
• Long-term visa: no administrative fee

If you apply electronically, the administrative fee must be paid by bank transfer via the payment details generator on the Information Portal for Foreigners.

Additional information

What is the benefit of this service

Notification of this event will fulfil the legal obligation. After this notification, the relevant document will be invalidated in the information system. The Ministry of the Interior will then issue you with a new residence permit or a duplicate visa sticker.
