Reporting changes of EU citizen with a registration certificate

Main information

As EU citizens, you are obliged to report certain changes to the Ministry of the Interior. Following this, you will be issued with a new registration certificate.

This applies to you if

You are en EU citizen and you will change the following:

  • name/surname,

  • marital status (marriage, divorce, widow/er/hood),

  • information in your travel document/identity document including its replacement,

  • information in your residence permit (e.g. address).

When to use this service

Report any change of name, personal status, travel document or identity document within 15 working days.

Report a change of address within 30 working days. You only need to report a change of address if you expect to stay at your new address for more than 180 days.

Report lost, destroyed, damaged or stolen documents within 3 working days.

What you need if you are using this service

  • You must report a change of your name or surname in a duly filled out formTo report this change, you must submit the following:
    • a travel document / an identity document stating the new name or surname;
    • registration certificate.

It is possible to report this change electronically, nevertheless the change of your name and surname must also be reflected in your registration certificate, and therefore a new certificate must be issued to you. Even if you report this change electronically, you need to come subsequently to an office ofthe MOI.

  • You must report a change of your personal status on a duly filled out form. To report this change, you must submit the following:
    • a document on the change of your personal status (such as a marriage certificate or a final divorce decree).
  • You must report a change of your travel document or identity document on a duly filled out form. To report this change, you must submit the following:
    • your new travel document/identity document.
  • You must report other changes on a duly filled out official form. In order to report this change, you must submit:
    • a document on the change.

Where and how to solve this service

You must report a change of your name or surname/marital status/travel document or identity card/other changes in your registration certificate to that office of the Department for Asylum and Migration Policy of the Ministry of the Interior (DAMP MOI), which is responsible for the district where your place of residence is situated.

Alternatively, you can send the filled-out reporting form, along with the documents, in accordance with the Rules of Administrative Procedure by mail to the office of DAMP MOI according to your place of residence. If you need to send the report of change from abroad, please send the filled-out reporting form with your documents by mail to:

It is always necessary to submit the originals or officially authenticated copies of the documents. Do not send the originals of the travel document. You will be asked to present the original when you receive the registration certificate.

If you want to report a change of marital status, you must present the original of the civil registration document. However, when sent by mail, it is sufficient to send a plain copy; you will be asked to present the original when you receive the registration certificate.

How much will you pay

To report the change of civil status and travel document is free of charge. All other changes are subject to a payment of 200 CZK. You can make up to two changes at once for this fee. The fees for all additional changes add up.

The loss, theft, destruction or damage of the registration certificate is subject to a fee 300 CZK. If you are reporting a loss together with changes, the fees also add up.

If you report the change in person, the administrative fee must be paid by credit card at a Ministry of the Interior office.

If you report the change by post, the administrative fee must be paid by bank transfer via the payment details generator on the Information Portal for Foreigners.

What is the benefit of this service

Your registration certificate will contain up-to-date information. Also, reporting the current address provides correct data on the number of inhabitants in the municipalities. Correct data consequently provide the overview necessary to optimize the territorial targeting of services for citizens such as waste collection, etc.

Failure to report a change of name or surname/personal staus/travel dokument or identity card/other changes in your certificate of residenceis a minor offence for which a finof up to CZK 3000 can be imposed on you on the spot.
