What you need if you are using this service
Send a notification describing all relevant facts and circumstances to the Inspectorate of Narcotic Drugs and Psychotropic Substances and report the theft to the Police of the Czech Republic.
Where and how to solve this service
You can send the form by post by registered letter with acknowledgement of receipt, or submit it in person, subject to an identification check, at the Ministry’s mailroom at the following address:

What you need if you are using this service
Send a notification describing all relevant facts and circumstances via databox to the Inspectorate, and report the theft to the Police of the Czech Republic. In the notification, please include all facts and circumstances that you are aware of.
Where and how to solve this service
Send the notification via databox (ID: pv8aaxd) to the Ministry of Health and report the theft to the Police of the Czech Republic. This procedure cannot be performed by email – even an email containing a recognised electronic signature.
