If you are a Disabled Person’s Pass holder, you must notify the regional branch of the Labour Office of the Czech Republic that issued the Pass of its loss, theft, damage or destruction.

To successfully notify the loss, theft, damage, or destruction of the Disabled Person’s Pass, it is necessary to fill out the Change Notification form.

Print out the Change Notification form in PDF format, fill it out with pen or typewriter, and sign it by hand. Then send the form by post or submit it in person to the address of the regional branch of the Labour Office of the Czech Republic that issued the Pass.

Jaký má služba benefit

In the event of loss, theft, damage, or destruction of the pass, upon your notification of this fact, the regional branch of the Czech Labour Office will issue a new Disabled Person’s Pass with a validity period identical to that of the replaced Pass. In such cases, health status will not be reassessed for the purpose of issuing a new Pass.
