Notification of loss, destruction, damage or theft of a residence card by an EU citizen or family member of an EU citizen

If your residence document is lost, destroyed, damaged or stolen, you must report the incident to a Ministry of the Interior office.

You have been issued with a confirmation of temporary residence or a temporary or permanent residence permit and your residence permit has been lost, damaged, destroyed or stolen. In these situations, you are obliged to report the matter to the relevant office of the Ministry of the Interior of the Czech Republic.

As an EU citizen or a family member of an EU citizen, you are obliged to report the loss, destruction, damage or theft of a document issued by the Ministry pursuant to Act No 326/1999 Coll. within 3 working days of the date on which the incident occurred.

You must report the loss, destruction, damage or theft of your residence document to the relevant office of the Ministry of the Interior of the Czech Republic. When a non-national reports the loss of their residence permit, a Ministry of the Interior employee will issue them a confirmation that the loss has been reported.

You must report the loss, destruction, damage or theft of your residence document to the Department for Asylum and Migration Policy at the Ministry of the Interior (OAMP MV).

Kolik budete platit

You must pay an administrative fee of CZK 300. 

If you report the loss in person, the administrative fee must be paid by credit card at a Ministry of the Interior office.

If you report the loss by post, the administrative fee must be paid by bank transfer via the payment details generator on the Information Portal for Foreigners.

Jaký má služba benefit

Reporting such an incident ensures compliance with a legal obligation. When you report the loss of your residence card, a Ministry of the Interior employee will issue you a confirmation that you have reported the loss The Ministry of the Interior will then issue you with a new residence permit to replace the one that has been lost, destroyed, damaged or stolen.

Možnosti odvolání

No appeals may be filed.

Sankce

Failure to comply with your obligation to report the loss, destruction, damage or theft of your residence document constitutes an offence.  You may be fined for this offence.
