Notification of any changes to the details on the Disabled Person’s Pass

If you are a Disabled Person’s Pass holder, you must notify the regional branch of the Labour Office of the Czech Republic that issued the Pass of any changes to the details on the Pass.

This applies to you if

Any of the details on your Disabled Person’s Pass are changed. 

When to use this service

Within 8 days of any changes to the details on the Disabled Person’s Pass, you must report these changes in writing to the regional branch of the Czech Labour Office.

What you need if you are using this service

To notify any changes to the details on the Disabled Person’s Pass, fill out the Change Notification form.

Where and how to solve this service

Print out the Change Notification form in PDF format, fill it out with a pen or typewriter, and sign it by hand. Scan the completed form and send it via a data box or by e-mail with an electronic signature to the electronic submission address of the contact point of the Labour Office that issued the Pass. What is the benefit of this service

If there are changes to the details on the Pass, when you notify this change, the regional branch of the Czech Labour Office will issue a new Disabled Person’s Pass with a validity period identical to that of the replaced Pass. In such cases, the health status will not be reassessed for the purpose of issuing a new Pass.
