As family members of EU citizens, you are obliged to report certain changes (please see below) to the Ministry of the Interior. Following this, you will be issued with a new residence permit. For some changes, you will subsequently be issued with a new temporary residence permit.

- name / surname,
- marital status (marriage, divorce, widow/er/hood),
- information in your travel document/identity document including its replacement or
- information in your residence permit.

You must report a change of your name or surname in a duly filled out form.
To report this change, you must submit the following:
- a travel document / an identity document stating the new name or surname;
- the existing certificate of residence permit or the existing residence card of a family member of an EU citizen;
- 1 photograph, if you are a holder of a residence permit or a residence card of a family member of an EU citizen.
It is possible to report this change electronically, nevertheless the change of your name and surname must also be reflected in your certificate of residence permit or your residence card of a family member of an EU citizen or your certificate of temporary residence for an EU citizen, and therefore a new certificate must be issued to you. Even if you report this change electronically, you need to come subsequently to an office of the MOI.
You must report a change of your personal status on a duly filled out form.
To report this change, you must submit the following:
- a document on the change of your personal status (such as a marriage certificate or a final divorce decree).
You must report a change of your travel document or identity document on a duly filled out form.
To report this change, you must submit the following:
- your new travel document/identity document;
- certificate of residence permit.
You must report other changes on a duly filled out official form.
In order to report this change, you must submit:
- a document on the change.
In case of a change of address, the new address will be marked on your travel document.

You must report a change of your name or surname/personal staus/travel dokument or identity card/other changes in your certificate of residence to that office of the Department for Asylum and Migration Policy of the MOI (DAMP MOI), which is responsible for the district where your place of residence is situated. You must report a change of address to that office of the Department for Asylum and Migration Policy of the Ministry of Interior, which is responsible for the district where your new place of residence is situated.
Alternatively, you can send the filled out reporting form, along with the documents, in accordance with the Rules of Administrative Procedure by mail to the official contacts of the MOI or directly to an office of DAMP MOI. It is always necessary to submit the originals or officially authenticated copies of the documents. Do not send the originals of the travel document and the certificate of residence permit by mail.

Kolik budete platit
To report the change of address and civil status is free of charge. All other changes are subject to a payment of 200 CZK.
The loss, theft, destruction or damage of the residence permit document is subject to a fee 300 CZK. If you are reporting a loss together with changes, the fees add up.
If you report the change in person, the administrative fee must be paid by credit card at a Ministry of the Interior office.
If you report the change by post, the administrative fee must be paid by bank transfer via the payment details generator on the Information Portal for Foreigners.

Jaký má služba benefit
Your temporary residence permit will contain up-to-date information. Also, reporting the current address provides correct data on the number of inhabitants in the municipalities. Correct data consequently provide the overview necessary to optimize the territorial targeting of services for citizens such as waste collection, etc.

Failure to report a change of name or surname/personal staus/travel dokument or identity card/other changes in your certificate of residenceis a minor offence for which a fine of up to CZK 3000 can be imposed on you on the spot.
