What you need if you are using this service

You may attend any municipal office of a municipality with extended powers, in Prague any office of Prague city districts 1 to 22, or a registry office or the nearest unit of the Czech Police, and report the loss, theft or destruction of the travel document or its finding. If you are abroad, the report should be made to the embassy of the Czech Republic. You may report the loss or theft of a travel document to the Ministry of the Interior if you are also requesting the issue of a passport within a period within 24 hours of a working day or within 5 working days.

In your report give your personal data, such as name, surname and date of birth, which fact you are reporting and which travel document it is the subject of the report.

Where and how to solve this service

The authority to which you made the notification will issue you with a confirmation of the loss, theft or destruction of the travel document. This authority shall ensure the entry of data on the loss, theft or destruction of a travel document in the agenda information system for the registration of travel documents.

What is the benefit of this service

Fulfilment of the obligation stipulated by the Travel Documents Act to immediately report the loss, theft, destruction of the travel document or its finding. Reporting the loss or theft of a travel document causes its expiry. Data on the loss, theft or destruction of a travel document shall be entered into the agenda information system for the registration of travel documents, from which these data are transmitted to the Schengen Information System, the Interpol database and are published on the Ministry of the Interior website at database of invalid documents. This will reduce the possibility of misuse of the travel document.

If the holder does not immediately report the destruction, loss or theft of a travel document, he commits an offence for which a fine of up to CZK 10,000 may be imposed.
