Reporting a change of name or surname

SERVICE DESCRIPTION

This is an obligation that you have, if you change your name or surname.

THIS APPLIES TO YOU, IF you change your name or surname. Under these circumstances, you are required to report this change to the appropriate office of the MOI CR.

SERVICE OUTPUT

By reporting a change of your name or surname, you will fulfil your legal obligation and you will receive a new residence permit card.

WHEN TO USE THE SERVICE

As an EU citizen or a family member of an EU citizen, you are required to report a change of your name or surname within 15 working days of the day, on which this change occurred.

WHAT YOU NEED IF YOU USE THE SERVICE ELECTRONICALLY

You must report a change of your name or surname in a duly filled out form. 

To report this change, you must submit the following:

  • a travel document / an identity document stating the new name or surname;
  • the existing certificate of residence permit, the existing residence card of a family member of an EU citizen or the existing certificate of temporary residence
  • 2 photographs, if you are a holder of a permanent residence permit or a residence card of a family member of an EU citizen.

It is possible to report this change electronically, nevertheless the change of your name and surname must also be reflected in your residence permit card. Even if you report this change electronically, you need to come subsequently to an office of the MOI.

WHERE AND HOW TO USE THE SERVICE ELECTRONICALLY

Send your filled out reporting form, along with the appropriate documents, in accordance with the Rules of Administrative Procedure to the official contacts of the MOI by e-mail with a recognised electronic signature, with all attachments being in the format of authorised conversion (via the filing office of the MOI CR), or to the data box (to DS MOI CR).

HOW MUCH YOU WILL PAY FOR THE ELECTRONIC PROCESSING

For notification of a change of name or surname, you will pay an administrative fee of CZK 200, which is associated with the issuance of a new document.

If you report the change electronically, the administrative fee must be paid by bank transfer via the payment details generator on the Information Portal for Foreigners.

WHAT YOU NEED IF YOU USE THE SERVICE IN PERSON OR BY MAIL

You must report a change of your name or surname on a duly filled out form

To report this change, you must submit the following:

  • the existing certificate of residence permit, the existing residence card of a family member of an EU citizen or the existing certificate of temporary residence
  • 2 photographs, if you are a holder of a permanent residence permit or a residence card of a family member of an EU citizen.

It is possible to report this change by mail, nevertheless the change of your name and surname must also be reflected in your residence permit card. Even if you report this change by mail, you need to come subsequently to an office of the MOI.

WHERE AND HOW TO USE THE SERVICE IN PERSON; WHERE TO SEND THE DOCUMENTS BY MAIL

You must report a change of your name or surname to that office of the Department for Asylum and Migration Policy of the MOI (DAMP MOI), which is responsible for the district where your place of residence is situated.

Alternatively, you can send the filled out reporting form, along with the documents, in accordance with the Rules of Administrative Procedure by mail to the official contacts of the MOI or directly to an office of DAMP MOI. It is always necessary to submit the originals or officially authenticated copies of the documents. Do not send the originals of the travel document and the certificate of residence permit by mail.

HOW MUCH YOU WILL PAY FOR USING THE SERVICE IN PERSON OR BY MAIL

Pokud oznamujete změnu osobně, správní poplatek je nutné uhradit platební kartou na pracovišti Ministerstva vnitra.

Pokud oznamujete změnu poštou, správní poplatek je nutné uhradit bankovním převodem přes generátor platebních údajů na Informačním portálu pro cizince.  

SANCTIONS

Failure to report a change of name or surname is a minor offence for which a fine of up to CZK 5,000 can be imposed on you on the spot.

LANGUAGE OF THE CASE

All submitted documents must be in the Czech or Slovak language or must be officially translated into Czech.
